Marriage
Please notify your employer promptly. Your current Eligibility Confirmation Document must be replaced. When you submit your notice, please also return your insurance card, Eligibility Confirmation Document, etc. if you have it. If you have dependents, please also return the insurance cards, Eligibility Confirmation Documen, etc,. of your dependents.
Please inquire with this health insurance association if you are a Voluntarily and Continuously Insured Person.
Procedure for getting married
| When you change your name |
Please submit to the HR as soon as possible, along with the marriage receipt certificate and insurance card, Eligibility Confirmation Document, etc., if you have it, in the "Insured Person (Family Member) Name Change Notice". The health insurance association will issue an Eligibility Notification, an Eligibility Confirmation Document, etc., card based on the notification from HR * Even if you use your maiden name in-house, you must notify the health insurance association.
* After December 2, 2024, new health insurance cards will no longer be issued.

Submit "Dependent Change Notification" within 5 days
* Please attach documents which crarify the eligibility as a dependent

Family (dependents) getting married When you get out of support |
Please submit a "Dependent Change Notification" within 5 days, and if the dependent has a health insurance card or an Eligibility Confirmation Document, please attach it to the form and submit it.
* There is no need to return your Individual Number Insurance Card.
